Connecting the Dots for Caregivers

Connecting the Dots for Caregivers celebrated National Caregiver Day on April 3rd by hosting its first Time to Talk caregiver connection and information event. We had a great turnout and thank all who helped to make the day a success!

Along with six other health care partners, Huron Perth Healthcare Alliance is excited to be a part of the Connecting the Dots for Caregivers project!

This project, funded by The Change Foundation, aims to improve the quality of care and experience for family caregivers, patients and health care providers in Huron Perth. 

Being a family caregiver can be rewarding and fulfilling but it can also be challenging and a significant source of stress and anxiety. This important initiative will bring together healthcare partners and family caregivers to co-create supports, programs, and resources that address the needs of family caregivers in order to ensure they feel valued, respected, engaged and supported in their important role.   

Partner Organizations include: The Huron Perth Healthcare Alliance, Alzheimer’s Society of Perth County, North Perth Family Health Team, One Care Home and Community Support Services, South West Local Health Integration Network and Star Family Health Team. 

To support this work, we are currently engaging with family caregivers and health care providers to hear their stories, understand their experiences and identify opportunities for improvement.
