Health Care Providers across Huron Perth Looking for feedback from Family Caregivers

Family Caregivers are invited to attend one of four public forums being held across Huron Perth. These forums are being held as part of Connecting the Dots for Family Caregivers - an initiative that is funded by The Change Foundation and aimed at improving the experience of those who support loved ones through their health care journey.

Family caregivers - family, friends, and neighbours who provide personal, social, psychological and physical support, assistance and care, without pay, for family members and friends in need for any period of time - are invited to join a forum and share their stories and experience with the health care system. These stories will be used to help identify areas for improvement that ensure family caregivers are heard, respected and valued.

“Being a family caregiver can be rewarding and fulfilling but it can also be challenging and a significant source of stress and anxiety.” says Anne Campbell, Executive Lead for Connecting the Dots for Family Caregivers. “This important initiative brings together six local health care partners who will listen to, learn from and act with family caregivers and health care providers to co-create supports, programs, and resources that address the needs of family caregivers and ensures they feel valued, respected, engaged and supported in their important role”

Community Forum Details:

  • August 1 - Listowel - Trillium Insurance Office - 495 Mitchell Rd S from 4:30 – 6:30 p.m.
  • August 2 – Goderich - Knox Presbyterian Church - 9 Victoria Street North, Goderich from 6:30 – 8 p.m.
  • August 14 - Clinton - Libro Credit Union - 48 Ontario Street from 5:30 – 7 p.m.
  • August 16 – Stratford - Rotary Complex - 353 McCarthy Road from 6 – 7:30 p.m.

“As a family caregiver for many years, I'm excited to be part of this project” says Rosemarie Richards. “This opportunity enables family caregivers to use their voice and share their experience, in order to bring about meaningful change in patient care and greater well-being for family caregivers.”

Health Care Partners of Connecting the Dots for Family Caregivers include The Huron Perth Healthcare Alliance (HPHA) (Lead Organization); The North Perth Family Health Team (NPFHT); STAR Family Health Team (STAR FHT); One Care Home and Community Support Services (One Care), the Southwest LHIN (SWLHIN) and Alzheimer's Society of Perth. The initiative is funded by The Change Foundation and is one of four Changing Care initiatives happening across the province.
