Celebrating 10 Years of the Community Stroke Rehabilitation Team!

HPHA's Community Stroke Rehabilitation Team (CSRT) celebrates their 10 year anniversary this month! Following a pilot project and with funding from the South West Local Health Integration Network (SWLHIN), this community model of integrated and individualized care for stroke patients in the Huron Perth District began in March of 2009. The SWLHIN is fortunate to have three of these teams including: Huron Perth, Thames Valley and Grey Bruce.

Since 2009 our Huron Perth team has served over 1,300 stroke patients with community stroke rehabilitation! Disciplines within the team include, Registered Nurse, Physiotherapist, Occupational Therapist, Speech Language Pathologist, Social Worker, Therapeutic Recreation Specialist and Rehabilitation Therapists.

Congratulations are extended to Grace Benedict, Registered Nurse and Erin Bickell, Social Worker who have been with the team since its implementation. We honour their dedication to the team and service to our stroke patients these last 10 years. Both Grace and Erin completed a Q & A feature about their roles. Click here to check out their stories!
