Assessment Services in Huron & Perth

If you’re worried you have COVID-19 or have been exposed to it, you should get tested – even if you don’t have any symptoms.

HPHA has Assessment Centres at three of our hospital sites.

You should make an appointment to visit an Assessment Centre if you:

  • have COVID-19 symptoms
  • do not have symptoms but are concerned you might have been exposed
  • do not have symptoms but think you are at risk (for example, if you are an essential or health care worker)
  • wish to visit a long-term care facility or retirement home

If you can, please bring your Ontario health card.

You may self-refer for a test or complete our Virtual Assessment Model (outlined below).

Please note that you must have an appointment for testing, we do not accept walk-in appointments.

Virtual Assessment Model

Please follow the steps below if you require an assessment.
  1. Complete the Ontario government’s On Line Assessment tool
  2. If the online tool tells you to seek clinical assessment, contact the office of your family doctor for a phone assessment. If you do not have a family doctor, contact Huron Perth Public Health. It is important that you do not visit your doctor’s office without calling first.
  3. If further action is needed, you will be given directions by the assessor.
