Complex Continuing Care

The Huron Perth Healthcare Alliance (HPHA) has complex continuing care beds at each of our hospital sites.

Complex Continuing Care (CCC) provides restorative care for individuals whose condition allows for functional improvement and evidence of benefit from continuing inpatient restorative, medically complex and specialized services, sometimes over extended periods of time.

Individuals may require CCC as a result of illness, injury, lifelong disability, chronic disease, or degenerative condition.

CCC is offered for adults who meet the eligibility requirements, have identified goals that are specific, measurable, realistic and timely and who have a discharge plan post CCC care and the individual’s goals / care needs cannot otherwise be met in the community.

Each patient has a personalized care plan developed to meet their unique needs and the interdisciplinary care team works in collaboration with the patient to achieve these goals.

Application and access for admission to CCC beds is facilitated through the South West Local Health Integration Network Home and Community Care Team. The Care Coordinator, the discharge team or physician will discuss the option of complex continuing care with you, when you are ready to be discharged from acute care or admitted from home.
