End of Life - Information for Caregivers is a guide for people who are preparing for the death of a family member, partner or friend. This is a difficult time for you. Take your time reading it. There is a lot of information to cover and it may be difficult to read some of the sections. Refer to the Table of Contents and start with the sections that are most important for you right now.

You may want to make notes about:

  • things you need to do
  • the decisions you need to make
  • questions you would like to ask:
    • your doctor
    • the other members of your healthcare team
    • your lawyer
    • family and friends
Your healthcare team is here to help you. Be sure that you ask about anything that is on your mind.

At the end of the guide, we have included a resources section with links to websites you may find helpful.
