The Role of a Transitional Caseworker

Crisis Intervention Programs support individuals in Huron and Perth Counties who are experiencing a mental health and/or addiction crisis. At the Huron Perth Healthcare Alliance our Crisis Intervention Programs include the following:
- Huron Perth Helpline & Crisis Response Team: Crisis assessments, brief crisis therapy, education about mental health and addiction services and treatment referrals via the helpline or in-person. The Helpline is available 24 hours a day, 7 days a week by calling 1-888-829-7484.
- Mobile Crisis Rapid Response Team (MCRRT): In partnership with Police Services, MCRRT provides immediate crisis de-escalation and connection to appropriate care. Requests for MCRRT can be completed through Police Services dispatch by calling 9-1-1.
- Transition Bed Program: The Transition Bed Program provides an appropriate response level to situational crisis to individuals who are experiencing mental health, addictions and/or criminal justice system involvement.
We asked Melissa, a Transitional Caseworker, to share some insights into their role as part of the Transition Bed Program.
In my role as a Transitional Caseworker, I assist clients experiencing situational crisis with my team. We develop a plan for crisis prevention, meet current needs and link clients to appropriate community services. My team provides short-term, recovery-focused support in a safe and therapeutic environment.
The Transition Bed Program is a goal-focused program. These goals are established by the clients and upheld by both staff and clients. This helps build connection and trust. This way I can learn more about each individual and the best way to provide support for them. My team works closely with Huron Perth Helpline and Crisis Response Team and the Mobile Crisis Rapid Response Team.
My role allows me to build a connection with clients and their community supports to help work together as a team to help achieve the goals of the client. I provide support for clients in different types of crises depending on their needs, and assist them in applying to treatment centres, completing housing applications, getting connected to counseling services, and more.
